IMPORTANT TERMS AND POLICY INFORMATION

Below you will find important terms of use, privacy policy, cookie policy and other privacy information that you should be aware of as a visitor to our site.

We are happy to announce that we are now able to take in person appointments. We do, however, have new procedures in place to make this possible so that we can keep our patients and staff safe.

We are requiring all staff, patients, and visitors to wear face coverings. We also request that everyone take their temperature prior to coming in. If you have a fever (100.4 or higher per CDC), cough, shortness of breath or difficulty breathing, chills, muscle pain, sore throat, new loss of taste or smell we ask that you reschedule your appointment. Please do not bring unnecessary companions to your appointment.

We are utilizing our online patient portal for you to complete necessary paperwork at home so that check in can involve minimal contact and minimal time in the waiting room. We encourage you to take any further steps you feel are necessary to maintain your health and safety including texting from your car that you have arrived and waiting there instead of the waiting room and/or rescheduling appointments you feel you would rather wait until a later date to complete.

We continue:

  • Enhanced cleaning protocols for our offices and equipment to minimize risk of infection to our patients and staff.
  • Refraining from unnecessary contact during all patient visits.
  • Limiting patient appointments and patient flow in our office to minimize patient-to-patient contact in waiting areas.

We will also continue to offer drive-thru and remote services and encourage all of our patients to utilize these methods first and only resort to in person visits if needs cannot be met otherwise. Drive thru and remote services include:

  • Supply pick-up, including wax guards, ear-tips, and batteries
  • Hearing aid repair services
  • Remote fine-tuning and counseling for patients

For any of these services, please call, email, or use the web scheduler to confirm availability and set an appointment.

We appreciate your patience and understanding during this time. Your hearing health is important to us, but it is only part of your overall well-being. Please remain safe and healthy, and take care of yourself by following government health professional advice, including:

  • Wash your hands frequently and effectively — for at least 20 seconds with soap and warm water.
  • Use hand sanitizer when hand washing is not an option.
  • Practice social distancing and avoid gatherings of 10 or more people.

Your Rights

• If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.

Get a list of those with whom we’ve shared information
• You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.
• We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

Get a copy of this privacy notice
You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.

Choose someone to act for you
• If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
• We will make sure the person has this authority and can act for you before we take any action.

File a complaint if you feel your rights are violated
• You can complain if you feel we have violated your rights by contacting us using the information on page 1.

• We will not retaliate against you for filing a complaint.

Your Choices
